A colleague of mine, a very experienced doctor, recently admitted he’d never come across ankle liposuction. He was a bit bemused by the whole idea. That surprised me, because here at Absolute Cosmetic it’s something we do commonly and for a lot of patients it’s the one procedure that finally deals with a problem they’ve hated for years.
So let’s clear it up: yes, ankle liposuction is a real procedure. Yes, it works. And yes, we perform it regularly.
Why do some people have heavy ankles?
Heavy ankles, what some people unkindly call “cankles”, come in two types and it matters which one you have.
The first type is structural. The heaviness comes from bone, muscle and tendon. Liposuction can’t change that, because there’s no excess fat to remove. If that’s you, an honest surgeon will tell you so at consultation rather than operate.
The second type is fatty. Some people simply carry stubborn fat collections around the ankle that won’t budge no matter how much they diet or exercise. This type responds very well to liposuction and it’s reasonably straightforward to treat with the right equipment and technique.
The only way to know which category you fall into is a proper physical assessment. It usually takes an experienced practitioner a matter of minutes to tell the difference.
Lipoedema and ankle cuffing
Part of the reason ankle liposuction has come to the forefront in recent years is lipoedema. Awareness has exploded; a lot of people are now recognising the signs in themselves from social media before they’ve ever seen a doctor about it.
Lipoedema is a condition where abnormal fat builds up in the legs, and it can cause more than just a heavy appearance. Some lipoedema patients develop what’s called ankle cuffing: the heavy tissue of the lower leg comes down and then stops abruptly at the ankle, creating a tight, painful band around the joint. Swelling then builds up around that cuff, and it can become genuinely uncomfortable; this isn’t just a cosmetic complaint.
Not every lipoedema patient gets cuffing, and you certainly don’t need lipoedema to have heavy ankles. But if you do have painful cuffing, liposuction to that area can relieve both the appearance and the discomfort.
How is ankle liposuction done?
The ankle is a delicate area, so the technique and tools matter more here than almost anywhere else on the body.
At Absolute Cosmetic we use the VASER ultrasonic liposuction system with a very fine 2.2 mm cannula, a thin, gentle instrument designed for sensitive areas like the neck and ankles where you have to work carefully around important structures. We also use a flat liposuction cannula around the ankle, which reduces the risk of waviness and rippling in the skin afterwards. That’s important, because the ankle has thin tissue and any irregularity shows.
Done properly, it’s a very good procedure with results patients are consistently happy with.
Is ankle liposuction safe?
Every surgical procedure carries risk, and the ankle deserves particular respect. The further an area is from your heart, the higher the risk of infection, and with any lower-leg procedure you also need to be mindful of blood clots.
That said, with careful technique and good aftercare, these risks are well managed. Diligent post-operative care, compression, movement, follow-up, prevents problems in the overwhelming majority of patients, and to date we haven’t had a serious complication from ankle liposuction at our clinic.
